The Clairmont Beaumont Provides a Team of Specialists Working Together For Their SETX Seniors

Clairmont Beaumont has brought together a dynamic group of Southeast Texas senior health professionals – nursing directors, nurses, Beaumont senior occupational therapists, and speech therapists who all are prepared play a role in the recovery of each Southeast Texas Senior.

The Clairmont Beaumont goal is to focus on each SETX seniors achieving the greatest functional outcome so that you can return home as quickly as possible – and “Get Back to Life”.

Your SETX Senior Physician

Each SETX senior’s personal physician may choose to provide medical direction during your stay at Clairmont Beaumont.

If your physician is unavailable to actively participate during your recovery, with your permission Clairmont Beaumont staff can update your doctor about your progress and plans for discharge.

SETX Senior Family

Family members and friends are important participants in each SETX Senior’s recovery.

Your family has a role in goal setting and providing the motivation and cooperation necessary for successful therapy.

Clairmont Beaumont’s staff helps your family members and friends actively participate in your recovery program.

Family knows you and often is able to assist you in ways even the best professionals cannot do alone- words of encouragement, sharing memories, and helping you achieve rehab goals.

Clairmont Beaumont Nursing Team

All nursing care during your stay will be guided 24 hours a day by Southeast Texas senior nurses who are experienced in rehabilitation and/or long-term care.

The nursing unit you are assigned to will have a charge nurse who will oversee nursing aspects of each SETX Senior’s care.

Supporting the charge nurse will be nursing aides who are primarily responsible for assisting you in daily tasks including personal hygiene, dressing, and eating until you are able to resume them yourself.

Southeast Texas Senior Social Worker

The Social Worker at Clairmont Beaumont (or his/her designee) will meet with you and your family soon after admission to collect a detailed history of your past living situation.

The social worker becomes your main contact. He/she will also work with you in planning for discharge back to your home or to a level of care that meets your evolving needs.

SETX Senior Dietitian

The Dietitian is specially trained to assure that each senior receives adequate and appropriate nutrition.

On admission, a diet history is taken to include your eating habits, previous special diets, and any nutrition related problems.

Food preferences are also noted.

Coordinating with your physician, nutritional needs are then followed throughout your stay.

Nutrition education may be provided with guidelines given for care at home.

Senior nutrition education can be particularly helpful if you’ve had extreme weight loss after an illness or hospital stay and for Southeast Texas senior diabetics.

Beaumont Senior Physical Therapist

Physical therapy is frequently ordered by your Southeast Texas senior physician to evaluate your physical capabilities and design a treatment plan to improve the quality of your:

• muscle tone
• coordination
• balance
• strength
• endurance
• joint flexibility.

Some of the techniques employed at Clairmont Beaumont  include gait and posture evaluation and training, therapeutic exercise, and wheelchair and pre-prosthetic management.

Dedicated Southeast Texas senior care staff will provide you with extensive education and training which will enable you to be as active and independent as possible- and to “Get Back to Life”.

SETX Senior Occupational Therapist

Your Southeast Texas senior physician may order occupational therapy during your stay to evaluate and provide treatment for

Occupational Therapy can be provided in the comfort of Clairmont Beaumont

the skills needed for independent living.

Southeast Texas senior occupational therapy often includes:

• grooming and hygiene
• eating skills
• homemaking
• money management
• upper body strengthening
• fine motor coordination
• thinking skills
• visual perceptual abilities
• community reintegration.

If needed, Clairmont Beaumont seniors will learn how to use prosthetic and/or adaptive equipment to assist you in work and leisure activities.

Carefully prescribed and supervised activities will facilitate your return to your highest functional level so you can get back to life.

Speech/Language Pathologist

Speech Therapy is available through Clairmont Beaumont

A Speech/Language Pathologist may work with you if you have any type of communication problem as a result of your illness or accident. These may include problems with speaking, listening, thinking, reading, writing, problem solving, sequencing, and organizing your thoughts.

The Speech/Language Pathologist is also trained to evaluate and develop treatment plans in the area of cognitive retraining for persons having difficulty with:

• attention
• orientation
• memory
• problem solving
• sequencing
• time management
• money management
• cognitive issues.

Clairmont Beaumont provides EXPERT assistance with Speech Therapy for Southeast Texas seniors

If you are having difficulty chewing or swallowing foods and liquids, the Speech/Language Pathologist will evaluate and develop a dysphagia (swallowing) management program to improve or manage your swallowing skills.
